How to Apply
How to complete forms
The forms below will open up in Microsoft Word and are set up so that you can click directly into them and fill in your information. See the steps outlined below for editing the documents.
-Open the desired form.
-You may click on the gray space in each section and type in the information requested.
-In the Technical Program Selection section, select from the drop down menus.
-To mark a check box, click on the box. To change your selection, click on the box again.
-If you would like to save a copy of the form to your computer, go to File -> Save As and select a location and name for your file.
-When all information has been typed in or selected, you may print the form.
-Click on the print icon at the top of the screen or select File -> Print.
-Submit the completed form(s), with all required signatures and dates, to either your current guidance counselor or to the Medford Vocational Technical High School Guidance Department.
