School Registration

Registering Your Child in the MPS

The Parent Information Center will need the following student assignment registration documents:

  • Registration form (DOWNLOAD FORM HERE)
  • Birth Certificate
  • Immunization Records (a school nurse will review the record to ensure compliance with Massachusetts Department of Public Health requirements)
  • Physical Examination (1 year old or less) with tuberculosis risk assessment/screening
  • Two forms for proof of residency (utility bill, or rental lease agreement, mortgage statement) (DOWNLOAD AFFIDAVIT HERE)
  • MA Transfer Card (if transferring from another school in MA)
  • Photo ID of parent/guardian
  • Personnel will be available for parents whose children need special education or bilingual/ESL services.
  • Parents of special education students should bring a copy of their child’s current I.E.P.

Personnel will be available for parents whose children need special education or bilingual/ESL services. Second Language students will be tested to determine if ELL services are needed.

Special Situations

Pre-school: In addition to the above, pre-school students are also required to provide

  • Lead screening results

Kindergarten: In addition to the above, kindergarten students are also required to provide

  • Physician documentation of stereopsis vision screening
  • Lead screening results

Grades 6-12: In addition to the above, students in grades 6-12 are also required to provide

  • Prior school transcript
  • Discipline record
  • Attendance record
  • Academic record
  • MCAS scores

If a student and parent are living with a relative that is a Medford resident, the following documents must be provided

  • Notarized living agreement form
  • Proof of residency for the person signing the living agreement

Students with special health care needs are recommended to bring in reports of doctors’ visits and evaluations.
